Medical Insurance 

We offer two medical insurance plans to meet different needs and preferences. See below for details on each option and what they cover. 

Option 1: Ascend to Wholeness-WebTPA PPO  

Ascend to Wholeness (ATW) is a self-funded PPO plan that uses the Aetna PPO network which includes Loma Linda University Health. ATW offers two coverage options: Accelerate and Access. Accelerate provides richer benefits with lower out-of-pocket costs, but members must meet certain point requirements. Access does not require participation, but comes with higher out-of-pocket costs.

Volunteers

You can use the following form with individuals who are volunteering at the church to ensure they understand they will not receive compensation for their work.

Volunteer Acknowledgement- Not an Employee

This form must be completed by any person who wishes to volunteer for or assist with a church position or activity such as, but not limited to Sabbath school teacher, Pathfinders, etc.

Short Term Travel Application

This form is a requirement for all church sponsored travel outside of Canada and the United States and is used for both individual and group travel. Completing this form will provide the traveling party with medical, trip cancellation, baggage, repatriation, and other forms of coverage during the time they are out of country.
