You can use the following form with individuals who are volunteering at the church to ensure they understand they will not receive compensation for their work.
Volunteer Acknowledgement- Not an Employee

This form must be completed by any person who wishes to volunteer for or assist with a church position or activity such as, but not limited to Sabbath school teacher, Pathfinders, etc.

Short Term Travel Application
This form is a requirement for all church sponsored travel outside of Canada and the United States and is used for both individual and group travel. Completing this form will provide the traveling party with medical, trip cancellation, baggage, repatriation, and other forms of coverage during the time they are out of country. Automobile Claim Notice
In the event that a vehicle owned by, or rented on behalf of the Southeastern California Conference, or a conference institution such as a school or church should be damaged, or cause damage to another person's property, you must download and complete the Automobile Claim Notice.
Liability
The Liability form is used in the event that property damage occurs at an institution owned by Southeastern California Conference, but the property damaged is not owned by SECC or an SECC institution. Liability coverage is not all inclusive and may or may not cover your specific claim. Please follow the directions included with the document and submit it to the appropriate party. Medical Payment Claim Form
Should a person other than a conference employee be injured at an institution owned by the Southeastern California Conference, or sustain an injury during an institution sponsored activity, the Medical Payment Claim Form should be completed.
Property
Property insurance covers damage to institutional property owned by the Southeastern California Conference. In the event that property damage has occurred, please print and complete the form, following the included directions.

If you are planning a move, or have already called our office to schedule a relocation, you will need to complete both the Relocation Information Form and the Inventory of Household Goods, and send them to our office as soon as possible. For your convenience, these forms may be printed and completed by hand, or they may be filled out on your computer and submitted electronically directly to our office.
If you live in an area of the country that has been identified by the USDA as a Gypsy Moth quarantine area, you must print the Gypsy Moth Inspection brochure and complete the self-inspection form provided in the brochure. Please make sure you have this form completed prior to our arrival at your residence. All, or part of the following states are considered quarantine areas: Connecticut, Delaware, District of Columbia, Illinois, Indiana, Maine, Maryland, Massachusetts, Michigan, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Rhode Island, Vermont, Virginia, West Virginia, and Wisconsin. For specific information on the counties quarantined, please refer to the Gypsy Moth Quarantined Areas - State and County List link in the Moving Instructions portion of this site.
